No senior should have to worry about not having food or shelter. Many seniors in poverty end up homeless because they simply do not have enough money to pay rent, even for the single room occupancy hotels (SROs) downtown. And virtually every senior we serve is at risk of homelessness due to the high cost of living in San Diego.
Our Transitional Housing Program (THP), a collaboration between Senior Community Centers, the City of San Diego, and the County of San Diego's Aging and Independence Services, provides intensive case management and ultimately, a new lease on life to seniors who otherwise might end up living on the street.
Our case managers work hard to identify at-risk seniors and get them the help they need. They are found at our meal sites, in cold-weather shelters, and through referrals from other community organizations and the San Diego Police Department.
Seniors accepted into the program agree to work with a case manager, create a work plan to meet personal goals, and follow some general program rules. In return, they receive fully-subsidized housing at a nearby SRO for up to 90 days in most cases and work on an individualized plan to prepare them for success in obtaining and maintaining permanent housing.
Many THP clients move to the Potiker Family Senior Residence, City Heights Square, or other affordable housing. The program has a 90 percent success rate as measured by the number of seniors who maintain stable housing for at least 6 months.
