Established in 1970, Serving Seniors helps San Diego County’s low-income seniors overcome poverty through whole-person, wraparound support including meals, housing, health and social services, and enrichment activities. We work at numerous sites across the county and in the homes of hundreds of homebound seniors. Our flagship Gary & Mary West Senior Wellness Center in downtown San Diego is unlike any other senior center in the nation. Bringing together a range of supportive services under one roof, the Senior Wellness Center operates 365 days a year and offers two floors of integrated care, including health and social services, housing navigation, enrichment activities, lifelong learning, a Cyber Café and linkage to over 25 onsite direct service partners, including a senior dental center. Last fiscal year, we provided over 1,400,000 meals and coordinated services to 6,800 older adults.

Job Purpose
The Health Education Lead is responsible for overseeing the provision of health assessments, case management, and health education to older adults accessing services at the Gary & Mary West Senior Wellness Center, supportive housing communities, congregate dining sites, and other locations throughout San Diego County. This role involves supervising two other Health Educators, providing program direction, and facilitating the growth of the program across all sites while ensuring quality client care and respecting client confidentiality.
Job Duties and Responsibilities
· Participate in the interviewing process for filling existing and new roles, ensuring the selection of qualified candidates.
· Supervise and provide guidance to two Health Educators.
· Develop standard operating procedures to ensure continuous improvement of existing departmental policies and processes.
· Provide direct services to seniors at various locations in San Diego County.
· Conduct individual health assessments, referrals, and case management based on client needs.
· Conduct follow-up phone calls to inform new clients of the Health Education program and monitor client progress.
· Contribute to the development and implementation of health education materials focusing on chronic disease prevention and self-management skills.
· Conduct activities to promote healthy lifestyles.
· Review and ensure linguistic and cultural appropriateness of materials and activities.
· Collaborate with community agencies to coordinate care and support.
· Provide advocacy and outreach in the community.
· Maintain a list of community contacts and referrals.
· Participate in multi-disciplinary case conferences.
· Ensure accurate data collection and documentation.
· Perform other duties as assigned.
Requirements
Required Qualifications
- Bachelor's degree preferred or equivalent combination of education and experience sufficient to successfully perform the accountabilities of the job; may require degree in specialized field, e.g., public health, health education or other related field.
- Experience providing one-on-one health education and delivering presentations in group settings.

Physical Demands and Work Environment
The ideal candidate must fulfill all physical requirements of the job with or without reasonable accommodation. While performing duties, the employee may be required to remain stationary for extended periods during client meetings and may be exposed to typical office environment risks.
Pay: $30 to $32 DOE
