Social Service Coordinator - Home Delivered Meals (HDM): San Diego


Job Title: Home Delivered Meals (HDM) Support Service Coordinator

Hourly Rate $18-20 DOE


The HDM Support Service Coordinator is responsible for communicating and coordinating nutrition, social service, health and mental health referrals for homebound clients that receive meals from the Home Delivered Meals program that are identified as needing additional assistance. The Coordinator works collaboratively with the Serving Seniors' Nutrition Program and also conducts community outreach in order to expand access and increase participation in the Home Delivered Meals program.



  • Conduct initial intake assessment, quarterly reassessments and annual nutritional risk assessment to determine eligibility for Home Delivered Meals recipients.
  • Ensure collaborative, interdisciplinary communication between Serving Seniors’ drivers, social workers and other Health & Social Services staff regarding older adults’ relevant clinical and/or social service needs.
  • Complete efficient, timely and accurate charting and notation of all client efforts and referrals.
  • Work with Serving Seniors’ HDM Drivers to introduce Union of Pan Asian Communities' (UPAC) Positive Solutions Program, and to conduct preliminary surveys regarding interest in Positive Solutions Program.
  • Meet regularly with UPAC Management staff to discuss objectives and program needs.
  • Ensure that the following metrics are satisfied:
  • 11 or more referrals per month are made to Positive Solutions Program. Referrals may come from ride-alongs, or from other methods;
  • 7 or more Provider Contacts are given to Positive Solutions Program Manager per month;
  • Complete and submit 11 or more PHQ-2 questionnaires for older adult participants served. This includes eligible participants served in congregate meal programs or case management programs.
  •  Ensure that the PHQ-2 survey is complete on a quarterly basis for all HDM clients and participate in quarterly community partner meetings with UPAC and Senior Management team from each organization.
  • Enter all outreach and/or screening activities into the UPAC tracking system and submit a written monthly report to Positive Solutions Program Manager which describes all activities and deliverables performed for that corresponding month.
  • Conduct community outreach to other community-based organizations and at other senior residences throughout San Diego County.


  • Bachelor's degree in Social Work or related field
  • Bilingual Spanish required
  • Minimum one year work experience/experience working with older adults preferred

While performing the duties of this job, the employee is regularly required to stand; walk; sit; use hands to finger, handle, or feel and talk or hear. The employee is frequently required to reach with hands and arms. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl.  The employee must occasionally lift and/or move up to 20 pounds.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.  

Work Environment:  

While performing the duties of this Job, the employee is regularly exposed to normal risks of working in an office environment (e.g., risks due to heavy computer use).  Must have valid California Driver's License, insurance and reliable method of transportation.
