Who We Are
Established in 1970, Serving Seniors helps San Diego County’s low-income seniors overcome poverty through whole-person, wraparound support including meals, housing, health and social services, and enrichment activities. We work at numerous sites across the county and in the homes of hundreds of homebound seniors. Our flagship Gary & Mary West Senior Wellness Center in downtown San Diego is unlike any other senior center in the nation. Bringing together a range of supportive services under one roof, the Senior Wellness Center operates 365 days a year and offers two floors of integrated care, including health and social services, housing navigation, enrichment activities, lifelong learning, a Cyber Café and linkage to over 25 onsite direct service partners, including a senior dental center. Last fiscal year, we provided over 1,400,000 meals and coordinated services to 6,800 older adults.

Job Purpose
The Case Manager - Advocacy & Community Resources works independently and collaboratively with a multidisciplinary team to provide high-quality service to older adults 60 and over throughout San Diego County. This role is critical in connecting older adults to both in-house and external community resources, offering outreach and community presentations, and providing advocacy and support for a range of social, health, and wellness needs.
Job Duties and Responsibilities
- Advocacy & Client Support: Provide personalized advocacy, helping older adults navigate systems to access essential resources, including healthcare, housing, nutrition, and social services.
- Outreach & Community Engagement: Conduct community outreach to identify needs, increase awareness, and expand access to programs like Home Delivered Meals (HDM). Deliver presentations on available resources to build community awareness and promote Serving Seniors’ programs.
- Independent Resource Connection: Work autonomously to connect clients to both in-house services and external community resources, ensuring each client receives a comprehensive support plan tailored to their needs.
- Health & Mental Health Coordination: Collaborate with Serving Seniors’ Nutrition and Health & Social Services staff to assess and address health and mental health needs, ensuring effective communication and coordinated support for HDM clients and other service recipients.
- Community Resource Navigation: Develop and maintain relationships with community partners to create a robust network of resources. Update and manage resource listings to ensure accurate referrals for clients.
- Caseload Management: Manage a caseload of clients via referrals, telephone and home-visits, coordinating resources and support to meet clients' needs effectively. Determine the best course of action to assist clients and exercise judgement to implement solutions.
- Interdisciplinary Collaboration: Regularly coordinate with multidisciplinary team members to ensure aligned support for clients as well as attend community meetings and stakeholder events as a representative of Serving Seniors.
- Documentation & Compliance: Complete timely and accurate documentation of all client interactions, assessments, referrals, and data entry using various software programs (Case Worthy, Teams). Adhere to budgetary guidelines and agency policies.
- Other Duties: Perform additional duties as assigned to meet the overall goals of Serving Seniors.
Requirements
Education and Experience
- Bachelor’s degree preferred. 2+ years of related experience required. Relevant fields include social work, gerontology, and human services.
- Bilingual in Spanish preferred: Ability to communicate effectively in Spanish is preferred to better serve the community.

- Technology: Proficiency in Microsoft Office, web-based systems, and client record management systems.
- Organizational Skills: Excellent organizational abilities for managing caseloads, documentation, and resource tracking.
Working Conditions
The ideal candidate must be able to complete all physical requirements of the job with or without reasonable accommodation. While performing the duties of this job, the employee is regularly required to remain in a stationary position for extended periods whilst meeting with clients and is regularly exposed to the normal risks of working in an office environment.
